A popular tenet created during the height
of Managed Care was to allocate health care resources to the minority
of the population that consumed the majority share of these resources.
While this tenet spotlighted the need for specialty programs such
as disease management, it has not yielded savings commensurate
with Managed Care's ambitious expectations. A more successful
approach to implementing disease management is based on real data
strategies. This approach is commonly known as Population-Based
Management (PBM).
Our approach to PBM is based on the Population-Based Care ProtocolSM.
A progressive step up from the one-size fits all approach common
to most disease management programs, this methodology reduces
cost by identifying and targeting the condition for which historical
trends of high claims are patterned. PBM creates a platform for
customized care management based on a proactive epidemiological
approach to screening and managing complex and chronic illnesses.
Within this methodology, Praxis applies sophisticated analytic
tools for predictive modeling to identify patterns of health
care consumptions at the individual level. Through analyzing this
data, we can create intervention initiatives to improve quality
of care outcomes such as preventative health screening and disease
management programs. This invariably translates to better management
of care for those suffering from a single or complex illness at
any stage of disease progression.
